On The Day Health & Media Consent Form

As a responsible tattoo studio, we want to ensure that not only we deliver exceptional work, but the healing process is not impacted negatively by any medical conditions you have. We of course provide comprehensive aftercare, but it’s important that you are fit, well, and not under the influence of alcohol / recreational drugs when you come in for your session. We also need to make sure that any prescribed, over the counter, or alternative herbal based medicines do not pose a risk to you being tattooed, or the healing process.

All information you provide will remain strictly confidential, and will not be disclosed to any third parties. We are required to retain information on you in case of any future disputes, however all records will be destroyed after 18 calendar months. Because the human body changes over time, you will need to complete this form for any new / future work you have done with us, to ensure that our records are as up to date as possible

This form should take approximately 5 minutes to complete. No data is stored by us until you submit your responses and you receive a confirmation at the bottom of your screen. A copy of your responses will be sent to the email address that you provide on this form. Please check your ‘spam’ folder if you do not see this in your inbox.

    REMINDER: Under UK Law, the legal age for a tattoo is 18. You must be aged 18 or over on the date of your studio time in order to be tattooed. Please bring ID with you to confirm this. If we suspect that you are underage and you can not prove your age, we will NOT tattoo you, and you will lose any deposit paid.

    We want to ensure that you are fit and well before commencing any treatment on you today. Whilst some of these questions may seem nonsensical, it's important that we ask them to ensure that we can provide our services to the best of our ability, and ensure the healing process of your tattoo is as seamless as possible.

    We are unable to tattoo anyone who is under the influence of drugs / alcohol. You will need to reschedule your appointment if this is the case, as tattooing someone under the influence can cause significant risks during both the tattooing and healing process.

    If you are applying your own numbing cream, please follow the instructions on the package. You should ensure that you apply this in plenty of time before your appointment. If you would like us to apply numbing cream for you, please arrive early for your appointment so that you don't lose studio time whilst waiting for it to become effective.

    We love showing off our work, however would never do so without your permission. We'd like to finish by asking you a few questions about how we can use your designs and completed tattoos across our website and social media channels.

    Acknowledgement & Consent

    Tattooing is normally a safe procedure, but as with anything involving the human body, there can always be risks. This can include infection, allergic reaction and scarring. If at any point during your tattoo session you feel ill, or any pain and discomfort above and beyond what you would normally expect, please let us know immediately. We would much rather take a break and ensure your wellbeing than risk injury to you because of any reaction to being tattooed.
